This form is used to create your listing the first time only.
If, part way through, you discover that you don’t have all of the information you need, you can click the Save and Continue Later link at the bottom of the form. You will be provided with a unique address that you can return to from any computer, and you can even choose to have the link emailed to you.
Note that the email can take a while so to be safe, copy or bookmark the link.
The name you provided when creating your user profile is the name that will initially appear on your listing details page (for reference, it also appears in the right corner of the black bar above this screen). If you wish to correct the way your name is displayed, choose Edit Your Login Details from the footer or click here.
If you want to update your listing after it has “gone live”, you simply view your listing while you are logged in and edit any of the information displayed in the fields provided.
You must be logged in as an approved practitioner to create a listing.
